Arise Release of Liability and Assumption of Risk

Please read carefully. By signing, you give up certain legal rights, including the right to sue Arise for injuries caused by ordinary negligence.

Arise programs may include hiking, walking on uneven or remote terrain, high altitude, camping or lodge stays, cold water, outdoor painting, breathwork, movement, group exercises, and working with horses on the ground [or riding].

I understand these activities carry risks that cannot be fully removed, including:

• slips, trips, falls, sprains, fractures and other injuries
• weather and environment: heat, cold, lightning, wildfire smoke, altitude sickness, dehydration, sunburn
• wildlife, insects, plants, allergic reactions and food-related illness
• remote locations where emergency help may be delayed
• travel to and from the venue
• emotional discomfort that can come up during coaching and reflective exercises

Horses. I understand that horses are large, powerful animals that can act unpredictably, even when calm and well trained. They may kick, bite, step on, push or knock down a person, startle at sounds or movement, or react to other animals. These are inherent risks of equine activities, and Arise cannot remove them. I will follow all handler instructions, wear closed-toe shoes, and stay out of the horses' kick and blind zones.

3. Health and medical care

• I confirm I am in good enough health to take part, or I have checked with my doctor.
• I have completed the Arise health questionnaire honestly, including any medical conditions, allergies, medications, injuries, pregnancy or mental health concerns that could affect my safety. I will tell Arise staff if anything changes.
• I understand every activity is optional. I may sit out or modify any activity, and Arise may ask me to sit out if it believes an activity is unsafe for me.
• I authorize Arise to arrange first aid and emergency medical care or transport for me if I can't consent myself. I am responsible for all medical, transport and related costs.
• I am responsible for bringing my own medications and any needed equipment, such as an EpiPen or inhaler.

4. Assumption of risk, release and indemnity

Assumption of risk. I take part voluntarily. I understand the risks described above, and others that can't be foreseen, and I knowingly accept them.

Release. To the fullest extent the law allows, I release Arise Coaching and Retreats, LLC, its owner Amanda Hope Kendall, and its staff, contractors, volunteers, horse owners and handlers, and venue owners (the "Released Parties") from any claim for injury, illness, death, or loss or damage to property arising from my participation, including claims caused by the ordinary negligence of the Released Parties. This release does not cover gross negligence or intentional misconduct.

Indemnity. I agree to cover the Released Parties' costs, including reasonable legal fees, for claims brought by me or on my behalf, or caused by my own actions, that this release covers.

This release binds me, my heirs, my estate and anyone who might claim on my behalf.

5. Conduct and general terms

• I will follow the Arise Code of Conduct and all safety instructions. Arise may remove me from an activity or program if my behavior is unsafe, as stated in the Terms and Conditions.
• I won't take part in any activity while under the influence of alcohol or drugs. Arise never serves alcohol. If a third-party vendor serves alcohol at an event, that vendor is responsible for serving it, and I am responsible for my own choices.
• Photo and video consent follows the Terms and Conditions. I can opt out on my registration form or by wearing an opt-out lanyard.
• This form is governed by the laws of the State of Idaho. If any part is found unenforceable, the rest remains in effect.
• This form stays in effect for this program and any later Arise in-person program I attend [within 12 months], unless I sign a new one.

6. Participants under 18

A participant under 18 may take part only if a parent or legal guardian signs below. By signing, I confirm that I am the participant's parent or legal guardian, that I have read this form, and that I accept its terms on the minor's behalf and on my own. I agree to the medical authorization in Section 3 for the minor.
